Information on shipping human remains to Romania - issuance of the Mortuary Passport

The Consulate General of Romania in Los Angeles issues the mortuary passport, necessary for shipping in Romania the human remaines of deceased persons, on the deceased family members or their authorized agents request, provided the next documents required by the related legal provisions are previously transmitted to the consulate:

  1. Statement describing the circumstances of the death, also requesting the issuance of a Mortuary Passport, written by a next-of-kin of the deceased person in Romanian;
  2. A copy of the Romanian passport of the applicant attached to his/her application for the mortuary passport issuance; if the funeral home is endowed with a notarized power of attorney from the family member of the deceased, it may apply on his/her behalf for the mortuary passport issuance, provided the application is transmitted to the Consulate General having attached the power of attorney in original and copy.
  3. Romanian passport and other Romanian documents of the deceased person – originals (birth certificate, marriage certificate Romanian ID card etc.) and copies of US documents;
  4. US death certificate (original US death certificate, issued by the public authorities at the place of death certified with the Apostille (please find details on the procedure of certification with the Apostille by visiting the website of the Secretary of State of the state which issued the death certificate; find out how to locate your Secretary of State by visiting www.nass.org); the original Death Certificate with Apostille will not be returned;
  5. Certificate that the body has been properly embalmed;
  6. Medical certificate that the human remains are not a communicable disease risk and that the cause of death was not a communicable disease issued by the competent Public Health Authority of the county;
  7. Burial transit permit/authorization;
  8. Certificate that the deceased person has a burial place in a cemetery in Romania;
  9. Self-addressed and pre-paid envelope to be used to return the mortuary passport.
